All of the effect of these other elements fades when you correct for APOB. Yes a high HDL:TG ratio is protective but the same way as having lower BP in the context of high APOB is protective. It speaks to your metabolic health. Top priority is maintain insulin sensitivity, control APOB the others are often a distraction.

New follower, very impressed with the preventative cardiology data, graphs, highlights etc!

As a vocal Canadian advocate for ASCVD prevention I’m finding a significant challenge in acceptance and implementation of Icosapent Ethyl (IPE) in spite of the Canadian Cardiovascular Society giving it the highest recommended possible (Strong evidence, High quality) and I believe it is because the focus is still only on cholesterol (and worse, LDLc vs ApoB) and the old school belief it’s a “clogged pipe” problem and not evolving into the “inflammatory disease” category. At the recent ESC Dr Deepak Bhatt presented on how the benefits of IPE in the REDUCE-IT trial was from restoring balance between pro-inflammatory Omega6 arachidonic acid (AA) and the anti-inflammatory (or more accurately pro-resolution) benefits of the Omega3 EPA.

Your recent piece also fails to mention the “unseen” risk of a hyper inflammatory state driven by high levels of insulin AND excess pro-inflammatory Omega 6 from our over consumption of soybeans and corn.

Just a thought for a future article.

I see that VLDL is one of the APOBs. Since a higher ratio of HDL:LDL has been said to be protective, does high HDL confer protection over VLDL. That started showing up on my chart years ago, but HDL has continued to be much higher, and Total within normal limits at the most. How do I understand the danger. PS, no insulin resistance or metabolic syndrome, but I can pinch an inch of what seems to be gas in my belly. I’m thin with a low BMI.
